Overview
Moodr Health is built for the parts of the health system that carry the most need and the least margin: Medicaid plans, rural hospitals, and safety-net providers. The platform supports care-plan enrollment, HEDIS and STAR performance tracking, and the workflows behind value-based reimbursement.
It started in the P3 program at Mon Health, where a single perinatal program piloted the approach and patients came to know the automated check-ins by name. From there it expanded to Medicaid plans, safety-net providers, and complex chronic care populations.
The premise is that patient engagement is not a soft metric. Done well, it pays for itself in quality scores and reimbursement.
